What is a dental implant?
A dental implant is a device that looks like a metal root
of a tooth and is placed and secured into the jaw by a specialist.
After the implant(s) are secure, the patient then returns
to us to complete the restorative procedure, which could consist
of a single tooth (crown), bridge, or full denture.

BLEACHING
INSTRUCTIONS FOR PATIENTS
Patient should receive technical report of CRA’s listing
pros and cons of bleaching.
In general, we usually bleach one arch initially so patient
can compare that arch with the opposing arch. (We usually
bleach the upper arch first.)
TECHNIQUE:
Brush teeth before bleaching.
Dispense a thin line of gel into bleaching tray, insert into
mouth being sure the gel covers entire tooth.
Try to wear two hours each day but can be worn for four hours
if no adverse symptoms.
Always re-apply bleach after each hour, as bleaching activity
diminishes rapidly after each hour.
If any sensitivity occurs to a tooth, rinse with the fluoride
bottle we gave you and/or do not bleach for a day. The sensitivity
will stop.
If gums become whitish or red around gumline, stop bleaching
for a day or so. This too will disappear.
You may want to rinse mouth after removal of bleach tray.

CRACKED TOOTH SYNDROME
You may have a very commonly occurring problem in one of your
teeth.
Teeth may crack when subjected to stress of chewing hard foods
or ice or by biting on an unexpected hard object. Teeth with
or without restorations may exhibit this problem but teeth
restored with typical silver alloy restorations are most susceptible.
SYMPTOMS AND SIGNS ARE SOME OR ALL OF THE FOLLOWING
1. Pain on chewing.
2. Pain on cold air application.
3. Unsolicited pain (usually leakage of sugar into tooth crack).
4. No radiographic evidence of problem.
5. No dental decay present.
6. Easy verification of crack when tooth is prepared for restoration.
TREATMENT OF CRACKED TEETH
1. SIMPLE CRACK: The majority of cracked teeth (about 9 in
10) can be treated by placement of a simple crown (cap) on
the tooth. When the tooth is prepared for the crown and a
temporary restoration is placed, the pain usually leaves immediately.
IF THIS IS THE CASE WITH YOUR TOOTH WE WILL PLACE THE FINAL
CROWN WITHOUT A PROBLEM ON YOUR NEXT APPOINTMENT AND THE CONDITION
SHOULD BE SOLVED.
2. COMPLEX CRACK: Occasionally (about 1 in 10) the tooth cracks
into the pulp (nerve) of tooth. IF PAIN PERSISTS AFTER PLACEMENT
OF THE TEMPORARY CROWN, YOU MAY HAVE A CRACK INTO THE PULP
OF THE AFFECTED TOOTH. PLEASE CALL US. THIS TOOTH MAY REQUIRE
ENDODONTICS (ROOT CANAL THERAPY) BEFORE THE CROWN IS PLACED.
This requires about two additional appointments before the
crown is placed.

We have placed crowns or a fixed bridge cemented onto your
teeth. They will replace your missing tooth structure or missing
teeth very well and give you years of service if you will
observe the following suggestions.
CHEWING: Do not chew hard foods on the restorations
for 24 hours from the time they were cemented. The cement
must mature for about 24 hours to have optimum strength.
RECALLS: Visit us at regular six-month examination
periods. Often problems that are developing around the restorations
can be detected at an early stage and repaired easily. Waiting
for a longer time may require redoing the entire restoration.
We will contact you when it is time for your recall appointment.
PREVENTIVE PROCEDURES: To provide optimum longevity
for your restorations and prevent future dental decay and
supporting tissue breakdown. Please use the following preventive
procedures that are checked:
A. Brush and floss after eating and before bedtime.
B. Swish vigorously for at least 30 seconds daily with one
of the following products: Colgate FLUORIGARD, Johnson &
Johnson ACT, LISTERMINT with fluoride. The best time is
immediately before bedtime (.05% neutral sodium fluoride).
C. Swish vigorously with 0.2% neutral sodium fluoride daily
before bedtime. (This requires a prescription from us.)
D. Use a 1.1% neutral sodium fluoride. (This requires a
prescription from us and a special tray for fluoride application.)
E. Use a Water Pik
F. Use a bridge cleaner as advised by us.
SENSITIVITY: Don't worry about mild sensitivity to
hot or cold foods. It will disappear gradually in a few weeks.
AGGRESSIVE CHEWING: Donot chew ice or any other hard
objects. Avoid chewing very sticky "hard tack" candy
because it can remove restorations.
PROBLEMS: If one or more of the following conditions
occurs, contact us immediately to avoid further problems.
A. A feeling of movement or looseness in the restoration
B. Sensitivity to sweet foods.
C. A peculiar taste from the restoration site.
D. Breakage of a piece of material from the restoration.
E. Sensitivity to pressure.
We have done our best to provide you with the finest quality
oral restorations available today. However, as with a fine
automobile or watch, only your continuing care and concern
can assure optimum service longevity.

INFORMATION ABOUT YOUR NEW COMPLETE DENTURES
We have done our best to provide you with well-fitted, functional,
and esthetic artificial dentures. We feel confident that after
a few weeks of becoming adjusted to the new dentures, you
will have years of satisfaction and use from them.
The following information will be helpful to you at this time:
1. YOUR FIRST FEW WEEKS: New dentures always feel strange
when first placed in your mouth. Several days or even a
few weeks will be required for you to feel accustomed to
them.
2. SORE SPOTS: Usually, your mouth will have a few "sore
spots" after wearing the dentures for 24 hours. Don’t
worry about these areas. They can be relieved with very
little effort on your next appointment. Another appointment
about 7 days later will usually eliminate any other sore
areas.
3. CHEWING: The new "bite" or occlusion will not
feel comfortable for a period of days. We will adjust the
contacting surfaces of your teeth after 24 hours and again
in about one week after the dentures have "settled"
into placed.
4. UPPER VS LOWER DENTURES: Your upper denture will rest
comfortably in place with moderate to strong "suction"
Although your lower denture will have good stability, it
is infrequent that "suction" can be expected on
a lower denture. We suggest that you avoid denture adhesives
unless you really have difficulty.
5. CLEANING THE DENTURES AND YOUR MOUTH: Your dentures can
be cleaned easily by using denture brush and mild toothpaste
such as "Complete" by Vick Chemical. Denture soaks
are also useful for the denture. Brush your gums with a
regular toothbrush once per day to toughen and clean them.
You may leave the dentures in or out of your mouth at night
depending on your preference. If they are out of your mouth
leave them soaking in water.
6. THE FUTURE: Your jaw bones and gums shrink up to 1/32
of an inch per year when your teeth are missing. This is
one of the main disadvantages of artificial dentures. Because
of this shrinkage, you should plan to have your dentures
and oral tissues evaluated once per year by us. We will
inform you when relining or rebasing of the dentures is
necessary. Wearing ill fitting dentures for too long without
refitting can cause severe bone loss and very serious oral
diseases.

dentures.
HOME USE OF FLUORIDE IN TRAYS
Because of your high need to prevent or control dental decay
(caries), or reduce tooth sensitivity, it is recommended that
you use specially custom fitted trays containing fluoride
daily. From research that is well documented and from our
own observation over many years, we know that use of these
trays and fluoride will prevent new decay almost totally and
will slow down currently present decay. Additionally, it will
desensitize a sensitive tooth’s root surface. However,
you must be consistent in carrying out the following described
procedure daily just before going to bed.
1. Cleaning Methods:
A. Brush your teeth well and floss them.
B. Use the described special cleaning methods that we have
recommended.

Place about six drops of Colgate Prevident into each tray,
spacing the drops equally. This is a medication requiring
a prescription. If you run out of the material, please tell
us so that we can give you a new prescription.
Place the tray(s) into your mouth seating them completely.
Expectorate the excess fluoride.
Let the trays and fluoride remain in your mouth for five
minutes.
Remove the trays, wash them with warm water (not hot water
as it will warp your tray) and place them upside down in
a convenient place to allow them to drain and dry before
their next use.
After Fluoride Use:
A. If you are living in an area that has fluoride in the
drinking water, rinse your mouth with water, expectorate
and go to be without eating.
B. If you are living in an area that does not have fluoride
in the drinking water, you need not wash the excess fluoride
out of you mouth. Expectorate the excess and go to bed
without eating.

YOUR NEW IMPLANT PROSTHESIS
The following information is important for you to understand.
- Your first
few weeks: Please chew slightly more carefully on these
restorations than natural teeth for several days. We
will adjust the bite (occlusion) on future appointments
until it is very well balanced. After you have adjusted
to the prosthesis, it should function in a very similar
manner as natural teeth.
- Cleaning
the implants and prosthesis: The gums and bone around
your implants require daily maintenance. Use a regular
toothbrush and other items such as: bridge cleaner and
dental floss, pipe cleaners, water pic, Rotodent, Interplak.
If cleanliness is not adequate, gum and bone degeneration
will occur. Please ask us about anything that is unclear
to you on the subject of cleanliness.
- The future:
We need to see you at least once every four months for
examination and professional maintenance. These appointments
are very important for the long term success of your
prosthesis. You should expect many years of use and
satisfaction from your prosthesis.

OCCLUSAL EQUILIBRATION
1. PURPOSE: There are many reasons why teeth and
jaws do not occlude (meet) in a tolerable position. Some
of them are: many fillings or bridges placed over a period
of years; orthodontics; developmental defects; oral surgery;
trauma; malocclusion (poor bite); bruxism; and clenching.
Occlusal equilibbration is the mechanical adjustment of
your teeth, dentures, bridges, fillings, or other oral appliances
to a position which allows your lower jaw to function in
a natural hinge in relation to your upper jaw without improper
influence from the teeth.
2. IS IT HARMFUL? Your mouth is being equilibrated
because some problem exists; pain; abnormal wear; breaking
of restorations; or other situations. The problem is usually
present because the teeth and/or restorations do not meet
in harmony with your lower jaw at the proper position. The
teeth and fillings have not "worn in" properly.
Occlusal equilibration "wears" some areas mechanically
and allows the teeth to meet harmoniously. It is not harmful
but is beneficial.
3. THE FUTURE: A simple occlusal equilibration can
be accomplished in minimum time and with only slight changes
in your occlusion (bite) occurring over a period of time.
More complex equilibrations may require several appointments
and the teeth may "shift" slightly between appointments.
When your symptoms are gone and your occlusion is relatively
stable, your equilibration will be finished. Obviously,
placement of any new fillings in your mouth will change
the way the teeth contact. The dentist accomplishing this
treatment should be advised of your past occlusion problem.
4. HOW YOUR TEETH FEEL: After occlusal equilibration
your occlusion (bite) will feel different to you. This is
to be expected. You will gradually accept this location
as your new chewing position, and it will feel very acceptable.
